Assuming you meet the medical criteria, you must decide if you can make and keep the following commitments.

• Keep all follow-up appointments.
• Use eye drops and medications as prescribed.
• Practice reading without spectacles in dim and bright light conditions.
• Openly and honestly communicate any issues or benefits you are experiencing with the AcuFocus Corneal Inlay.
